Jacob initially presented with the chief concerns of crowding, with an ectopically erupting right maxillary canine, and supernumerary maxillary lateral incisors. The treatment plan involved fixed braces, the removal of the palaltally positioned supernumeraries, and the goal to transpose the right maxillary canine and lateral incisor (through distalization of the lateral thus allowing the auto eruption of the canine in the mesial position). He was treated in approximately 4 years, and finished with excellent occlusion and very good esthetic results. Some composite bonding of the lateral incisors as well as some reshaping of the canine in the lateral position was performed to enhance the esthetic outcome.
